Information We Collect

The form is divided into sections to help us understand your needs:

  • Personal & Contact Info: Basic details and dates of birth for you and your spouse.
  • Income & Social Security: Details on your current work, pensions, and Social Security filing status (Note: We do not ask for your Social Security Number).
  • Retirement Accounts: Approximate market values for your IRAs, 401(k)s, and other retirement vehicles.
  • Fixed Assets & Property: Information on annuities, life insurance policies, and real estate.
  • Expenses: Your desired monthly retirement income and average monthly expenses.
